Contract Agreement

I understand filling out this form is a contract between the members of the PAC and Buehler Rules Inc. Buehler Rules Inc has a set-up fee for Oregon State PACs of $500. Treasurer Services will be charged at a rate of $100 per hour there after, billing on a monthly basis. The $500 Start-Up Fee Includes A. Register PAC with SOS B. Obtain EIN from IRS C. Set-up Bank Account D. Create Anedot Account E. 1st Bank Deposit transaction All payments are due upon receipt. Invoices over 2 weeks past due will incur a fee of an additional 3% monthly. Treasurer for your State Committee will be Christina Buehler. She will file all transactions in a timely manner. I understand when creating a committee my name, public contact information, employer name, city and state will be publicly available. I understand I am responsible for submitting receipts and all transactions to the treasurer on a regular basis and will billed for any fee's incurred to Buehler Rules Inc. that are the fault of the organization and not the Treasurer. I understand after the bank account is created, I am required to contact the bank, add myself as Director and obtain my own debit cards or book of checks. I attest to the information below to be true and accurate to the best of my knowledge.

How Many Directors

You may have multiple Directors. All will be added to the bank account as signers. You may also add signers for the bank account that are not directors.
